A favorite theory among archeologists and medical doctors is he may have had Marfan’s syndrome which tends to result in very tall men with feminine features. Many are sterile but some have had children, which we know now that Akanakton did. There are a couple of other medical conditions including being intersexed that could have resulted in some of these features but he fits Marfans the best at least that is what I have heard so far. Now there is a theory that his "co-pharoh"after Nefetiti “died” may have been Nefertiti using a male throne name or he may have been a genetically related cousin and son-in-law married to Akanaton’s daughter - Semkare, also is shown with feminine features though again that might have been just to make him look like his co-ruler.
